Fuse the 2 font substitution lists into a single one

We do have 2 parallel font substitution lists:
media\inf\font.inf
base\setup\lib\muifonts.h

hbelusca explained, that the only reason for that are historic reasons. No technical reasons.
To eliminate them becoming offsync this task is aiming to fuse both lists into a single one.

If it's not more complicated, I would vote to keep the font.inf, because it is a bit easier to place comments into that one than it is for the .h file.
