io: Crash when plugging the same USB device 2 twice

    Description

      How to reproduce this bug:

      1- Plug your USB device and the unplug it
      2- Plug it again

      ReactOS should crash in the same way it did for me.
      I tried with different devices, USB pen drive and USB mice, with the same results.

      Debuglog is attached.
      Tested with revision 57156 and VBox 4.1.20
