DOS Box won't starts but main window is messed up

I try to launch DOS Box and it starts up but the main windows won't display any video and displays what ever is behind it (I attached a screen shot). I also included a debug log. I tried playing Star Trek 25th Anniversary for DOS using NTVDM and it works except for the mouse so I tried to use DOS Box.
