Explorer toolbar has gray square buttons when disabled

This patch fixes the problem by assigning a custom disabled imagelist by generating a grayscale version of the normal bitmap when initializing the toolbar.

This is more of a workaround, in theory the toolbar control from comctl32.dll should provide a default disabled bitmap for the ILC_COLOR32 code path, now it seems that fails to mask them.
